Hazel Patricia Eubanks was born January 15, 1951 in Green Cove Springs, Florida. The early years of Hazel’s life were spent in St. Augustine, Florida until the age of thirteen, when she moved with her family to Columbus, Ohio. She attended Columbus Public Schools and graduated with the Class of 1970 from Linden-McKinley High School. An active supporter and participant of the civil rights movement, Hazel was one of many Eubanks family members who marched alongside Dr. Martin Luther King and Andrew Young. The majority of Hazel’s career was in the public sector. During her fifteen years of employment with the State of Ohio, she held positions within the Department of Health, as well as the Department of Commerce, Division of Liquor Control. Hazel also spent a number of years working for the Columbus City Schools. Her employment career ended with a job as a Pharmacy Technician for The Kroger Company. Hazel lived her life to the fullest. She was well-known for Bid-Whist card playing, her cooking, and her love of music and traveling. On December 31, 2016, at the age of 65, Hazel was called from labor to reward while at Mt. Carmel East Hospital. She was preceded in death by her mother Leonia Eubanks Thomas, father James Clements, Sr., stepfather Alto Thomas, Sr., sister Jacqueline Bannerman, brother Alto Thomas, Jr., grandmothers, Estelle Eubanks and Lillie Mae Clements, and grandfather Leon Eubanks. She is survived by her daughter India Eubanks, and grandchildren, Imani Carroll and Malik Holder, all of Columbus; stepmother Ethel Lee Clements; sisters, Beverly Radford of Columbus, Lena Smith and Jacqueline Williams, both of Jacksonville, FL; brothers, James Clements, Jr. (Yvonne), Frank (Norma) Clements, Sylvester Clements, and Kelvin (Tammy) Clements, all of Jacksonville; brother-in-law Curtis Bannerman of Brooklyn, NY; aunt Carol (Bernard) Mitchell of Tampa, FL; uncles, Joe Eubanks, Solomon Eubanks, both of St. Augustine, Thomas Eubanks of Sarasota, FL, and Charles Eubanks of Brooklyn; lifelong, loving, best friend and sister of 44 years, LaiMari “MiMi” Bailey; other special sister-friend Veronica Westbrook; and a host of nieces, nephews, cousins, other relatives, and friends.
